1. From the Home screen, swipe up to access the apps screen.
2. Go to:

4. General management
5. Reset
6. Tap
7. Reset network settings
8. Review the info then tap
9. Reset settings
10. If presented, enter the current PIN, password, or pattern.

To confirm, review the info then tap Reset

Make sure that you have all updated software and double check your settings. It i s possible that you are having antenna issues or other hardware issues. You may have to open your phone and check the antenna. Sometimes issues like this are resolved by replacing the charging port that has the antenna connections.

Since ifixit does not yet have a specific guide for this, use something like this video Replacement boards are relatively inexpensive and are readily available at many online stores.

While you work on your phone, take lots of pictures. Once you are done with it, take those pictures and create a guide for iFixit.
