Better to open up the PS4 slim and take the PSU out to get the ADP-xxxXR model number.

Thanks mate… really appreciate… but i cant find the model number which you've shared. Im from india. Is that model is not available over here? In amazon too it's not available.

Have you gotten the model number from under the PSU? I'm in Australia so I don't think I'll be much help here with suppliers.
Someone on here from India might be able to point you in the right direction for suppliers.
