Peel the adhesive backing off of the new rear glass, carefully line up one edge of the glass against the phone chassis, and firmly press the glass onto the phone.

Remove the thirteen 3.5mm Phillips #00 screws securing the midframe to the phone.

Firmly grasp the edge of the midframe assembly.

Lift up on the midframe assembly while pushing down on the battery to separate the midframe assembly from the rest of the phone.

It’s annoying that the rear glass adhesive is not included in the list of parts required for this procedure, but is listed on a separate page. I bought the kit and the item listed under “Parts” above, the front facing camera. When I received the stuff and went to do the deed, then I saw that I needed the adhesive for the rear glass. I had to buy it and paid more shipping. Fercrissakes, if it’s gonna be required, include it in the “Parts” list so I only need to buy things once!
