Data services aren't working. Says Airplane mode is on, but isn't!

This problem has been ongoing for sometime now, any time my phone dies, or resets, I have a No Signal sign on when it reboots. When trying to make a call, it says my Airplane Mode is on, but it isn't.

I've got it working before by resetting the phone multiple times, but that method is no longer working.
