speaker and mic works but still cannot hear when making calls.

When I make or receive calls I cannot hear the other side and times they cannot hear me either and also for some odd reason it automatically goes to speaker mode. I can make calls on skype it works flawlessly even the speaker works when playing music. I looked every where yet no answer. Is it a software issue or a hardware and what can i do about it?
